Birth and Death Certificates
How to Request.
Birth and death certificates are confidential documents; therefore,
- only the individual the biological or adoptive parents of the individual
- the biological or adoptive grandparents of the individual
- the guardian or temporary guardian of the individual
- a legal spouse, next of kin, or personal representative of the individual’s estate
- an adult child of the individual can request and obtain the birth certificate.
If you were born anywhere in the State of Georgia, you may request a certified copy of your birth certificate from the Probate Court of Butts County. If the decedent passed anywhere in the State of Georgia, you may request a certified copy of the death certificate from the Probate Court of Butts County.
Vital Records requests are fulfilled in the office any time during regular operating hours and can be made in person, by mail or email.
What do I need to bring?
- Valid photo ID. To obtain a birth certificate or death certificate you must present a Georgia Driver's license or State of Georgia Identification Card unexpired or expired for not more than one year, State of Georgia Weapons Carry License, unexpired Driver's License or Identification Card issued by another U.S. State, jurisdiction or territory, unexpired U.S. Passport, unexpired Foreign Passport, U.S. Military Identification, Military Dependent Identification, Veteran's Identification, unexpired Consulate Card, Transportation ID, Debit Card with Picture and Employer ID Card.
- Payment. The cost is $25 for a certified copy and $5 for each additional copy received at the same time. The Court accepts cash, money orders (exact amount) or credit or debit cards for a small fee.
Can requested records be mailed?
Yes. For requests via mail, or email the Probate Court will need the following information provided:
- A copy of your valid photo ID
- Mailing address of where the certified copies should be sent
- The name of the person of whom you are requesting the record for
- Your relationship to that person
- The type of record needed (birth or death)
- For birth certificates: the date of birth and the mother's name
- For death certificates: the date of death
- County in which birth or death occurred
- How many copies you will need
- Payment by Money Order or Cashiers Check. No Personal Checks!
